DEADLINES OF ACCEPTANCE OF ACCEPTANCE OF PROGRAMS SHOULD BE CONSIDERED BY THE HEALTH SCIENCES INSTITUTE 30 – 31 JANUARY 2018. (REGISTRATIONS TO BE PERFORMED).
DOCUMENTS REQUIRED TO REGISTER

1. Graduate Diploma or Provisional Graduation Certificate (e-government output / notarized copy / original) for the Ph.D. Program, a diploma or temporary graduation certificate (e-government output /
2. Approved Undergraduate / Masters Transcript (original / notarized copy) (Document will remain in the Institute)
3. Graduate Education Exam (ALES) Result Certificate is original
4. YDS Final Document (for Ph.D. Program)
5. Photocopy of identity card
6. Military status document (e-government output)
7. Certificate of residence (e-government output or mukhtar)
8. 4 photos
9. A bank receipt showing that the tuition fee for the relevant program has been paid
10. Bank receipt showing course registration fee for scholarship prep students and special students
11. Special student application petition for applicants as a special student
12. Online Application Form

Note 1: There is absolutely no registration with missing documents.
Note 2: The student who is in Baka / Leak is not registered.
Note 3: Color copier / photocopy is not accepted.
